How Orthotic Bracing Is Used to Treat Scoliosis
An overview of scoliosis — causes, warning signs, and health risks — and why orthotic bracing combined with physical therapy is today's safest, most effective treatment.
Scoliosis is a condition in which the spine curves abnormally to one or both sides, forming a C- or S-shaped curve instead of a straight line. It can occur at any age but is most common in children and adolescents, with roughly 85% of cases having onset during puberty with no identifiable cause; the remaining 15% are linked to factors such as musculoskeletal disease, injury, congenital defects, flat feet, or poor sitting posture.
Some visible warning signs include uneven shoulders, one shoulder blade protruding higher than the other, an asymmetrical gap between the elbows and waist on each side, a tilted or rotated pelvis, and unevenly folded skin creases on the back. Left undetected and untreated, scoliosis can progress silently and lead to serious consequences: abnormal posture and gait, disc and joint degeneration, and in severe cases, effects on the rib cage, lung capacity, breathing function, and cardiovascular health.
Today, orthotic bracing combined with physical therapy exercises is considered the optimal treatment for scoliosis, delivering strong corrective results while being non-invasive, painless, and carrying far fewer side effects than surgery. This is consistent with findings from the BrAIST trial published in the New England Journal of Medicine (2013) in adolescents with Cobb angles of 20–45 degrees: treatment success — avoiding surgery — reached 72% in the braced group versus 48% in the observation-only group, with success climbing markedly with wear time, reaching 90–93% among those wearing the brace more than 13 hours a day. A brace works by correcting and holding the spine in a straight position, preventing the curve from progressing further during puberty, while also being adjustable as the patient continues to grow.
Treatment outcomes with bracing depend on several factors: a milder curve with its apex lower on the spine responds better and faster; earlier treatment age, while bone is still immature and pliable, yields better results; the patient's consistent cooperation throughout treatment is decisive; and finally, the quality and craftsmanship of the brace itself — which demands specialized skill — directly affects the final outcome. The brace-making process at the clinic typically follows four steps: examination and X-ray to assess curve severity; plaster casting to take a mold directly from the patient's body; refining the mold and manufacturing the brace; and finally, instruction on how to wear it, the usage schedule, and specific supportive exercises for each case.
