Hydraulic prosthetic knee joints deliver a more natural gait, greater stability on uneven terrain, and — per research on microprocessor/hydraulic knee technology — a measurable reduction in falls and fear of falling.
A hydraulic-controlled prosthetic knee is a component used in prosthetic legs for people with above-knee (transfemoral) limb loss. It works on a hydraulic principle, using fluid — typically oil — to regulate knee joint movement, allowing users to move more naturally and safely.
Compared with simple mechanical knee joints, hydraulic knees deliver a smoother gait because they can flexibly adjust the speed of flexion and extension, more closely simulating natural knee movement — a benefit that matters especially for new prosthetic users. They also improve stability on uneven terrain such as slopes and stairs, or when changing walking speed suddenly, thanks to better control over force and speed.
Research on microprocessor- and hydraulically-controlled prosthetic knee technology has shown meaningful benefits for limited community ambulators: a systematic review and meta-analysis of 13 studies covering 2,366 patients, published in Disability and Rehabilitation (2021), found a statistically significant reduction in falls among users of advanced-control knee joints compared with mechanical ones. This helps explain why hydraulic knees are increasingly favored for users with moderate-to-high activity levels.
Hydraulic knees also distribute load more evenly and absorb shock during walking, reducing strain on the rest of the body — hips, back, and the remaining sound leg — which lowers the risk of secondary injury over time. They suit a wide range of activity levels, from gentle walking to higher-intensity activity depending on the specific model, once the user has been properly assessed and fitted.
Fitting a hydraulic knee is a multi-step process, from initial assessment and consultation, through socket and trial-fitting fabrication, to installation, alignment, fine-tuning and final delivery with scheduled follow-up visits. The joint also needs proper care: keeping it clean and dry, periodic inspection and oil changes (every 1–2 years), avoiding hard impacts, and respecting weight limits.
