Prosthetic Socket Design Technology: The Foundation of an Effective Prosthesis

The socket is the part of a prosthesis that fits directly around the residual limb after amputation, responsible for transferring body weight and force from the limb to the prosthesis, which is what allows an amputee to control the device while moving. Because of this critical role, socket design is considered the single most important step in fabricating a complete prosthesis, before it is assembled with other components such as the foot, joints, pylon and cosmetic cover.

One notable advancement is the Silcare Breathe Cushion Liner, a proprietary silicone liner from Blatchford–Endolite featuring perforations across its entire surface that let sweat escape the socket efficiently — particularly useful in a hot, humid climate like Vietnam's. This type of liner is often paired with Vacuum-Assisted Socket Suspension (VASS), a technology now widely used in developed countries such as the UK, the US and across Europe, especially among active users who take part in high-intensity sports like running, tennis or table tennis.

In terms of scientific evidence, studies comparing vacuum-suspended sockets with traditional pin-suspension sockets suggest that vacuum suspension tends to better stabilize residual-limb volume and reduce pistoning — the sliding of the limb within the socket during walking — though results vary between studies and should be understood as a general tendency rather than an absolute guarantee. Notably, Lê Ngọc Hoàn, MSPO, founder of Lê Hoàn Prosthetics & Orthotics, published his own research on a running prosthesis using a VASS system in JPO: Journal of Prosthetics and Orthotics (2021), finding clear benefits in comfort and residual-limb volume stability for runners, even though running speed did not differ significantly from a non-VASS prosthesis.

Alongside the development of modern sockets, prosthesis users today can not only walk comfortably but also participate in sports such as badminton, table tennis, tennis, running, jumping, or carrying heavy loads — activities that older socket designs struggled to support. As a result, at competitions such as the Paralympics, the running and jumping performance of many lower-limb amputee athletes using modern prostheses has approached that of able-bodied professional athletes.
